prepares and empowers all children in our community to care for their oral health.
Job Purpose and Impact
The Community Health Worker (CHW) plays a critical role in our school-based oral health program. This
role touches every aspect of our work with the community from outreach to clinical and educational
services, making sure our children will get the services they need and deserve.
Key Accountabilities
- Community Outreach – Serve as the liaison to assigned schools, have a working relationship with the principal, school nurse, social worker, and community liaison and attend school events like conference days, back-to-school nights, health fairs, etc.
- Class Education – Prepare for and teach Ready, Set Smile oral health curriculum in the school settings
as well as maintain appropriate records of these tasks - Clinical Services – set up clinic equipment and supplies at the school and work as team with all dental
providers to ensure that patients have comprehensive and coordinated care, including assisting licensed
providers. - Administrative Services – support the Team Leader with insurance submissions, patients records and
our systems, ensuring accurate documentation. - Other duties as assigned.

Position Information
- Local travel is required.
- Relocation assistance will not be provided for this position.

Qualifications
- High school diploma required, postsecondary education (Associates, Bachelors) preferred.
- A Community Health Worker certification is required (and Ready, Set, Smile will pay tuition for the
incumbent to receive the certification through Normandale’s online curriculum). - Basic computer skills.
- Experience working in a multicultural setting.
- Strong communication and organization skills.
- Ability and willingness to provide emotional support, encouragement and motivation to patients
