The CDCâs Community Health Workers for COVID Response and Resilient Communities (CCR) initiative was launched to mitigate pandemic-related health inequities by supporting Community Health Worker (CHW) deployment across disproportionately affected populations nationwide.
This study evaluated the implementation and impact of CHW-led services across CCR-funded recipient organizations. Using a standardized, participatory evaluation framework, researchers assessed how CHWs delivered health education, conducted outreach, facilitated access to clinical care, and addressed social determinants of health (SDOH) in marginalized communities.
