The Minnesota Community Health Worker Alliance and the Minnesota Department of Health invites CHWs, employers, partners, and stakeholders to participate in the Annual Sustainability Roadmap Update on September 10, 2026, from 12:00 to 1:30 p.m. via Zoom.
The Sustainable CHW Roadmap was developed through a statewide, community-driven planning process to strengthen and sustain Minnesota’s Community Health Worker workforce. The roadmap is supported by four implementation workgroups focused on Sustainable Funding & Policy, CHW Models Implementation, Statewide CHW Measurement, and CHW Training & Workforce Development.
During this virtual update, each workgroup will share progress made over the past year, highlight key accomplishments, and discuss priorities and planned activities for the next 12 months. This is an opportunity to learn about ongoing efforts to expand and strengthen the CHW workforce in Minnesota, connect with stakeholders from across the state, and identify ways to get involved in advancing the roadmap’s goals.
