Shaping the Future: A CHW-Led Movement to Achieve Sustainability

Envision, a community health worker (CHW)-led Training and Technical Assistance Center, delivered an 18-month sustainability intervention for health departments funded through the Centers Disease Control and Prevention’s Community Health Workers for COVID Response and Resilient Communities (CCR). Twenty-two recipients began; 20 completed at least half of the sessions, and 18 set sustainability goals. An in-person summit engaged 437 participants (54.7% CHWs) from 63 recipients. Participants reported increased familiarity with funding models, greater confidence in sustainability planning, and early progress toward long-term CHW workforce sustainability goals.
