Lunch & Learn Session
At this year’s HCH Learning Days the MDH CHW Initiatives, MDH Center for Health Promotion, MDH Health Care Homes, and the Minnesota CHW Alliance collaborated to host a CHW Lunch and Learn session.
The interactive, peer-learning lunch session brought together CHWs and CHW program managers/supervisors to learn from one another and map how CHWs are integrated within Health Care Home clinics, healthcare systems, CBOs, and the broader care delivery landscape. Participants used a new CHW Model Documentation Tool, developed through the Minnesota CHW Roadmap for Sustainable Infrastructure, to document their program components and learn from peers in real time.
The meeting supported a shared understanding across:
- CHW roles and services within HCH certification standards and models of care
- The teams and organizational structures that support CHW work
- The program, patient, and clinic outcomes CHWs contribute to
