Health Equity: Many Voices, Shared Vision

Overview

Our one-day conference will feature an opening plenary featuring Health Commissioner Ed Ehlinger and DHS Assistant Commissioner Anne Barry. We will have two sets of concurrent breakouts, a plenary health equity panel, discussion breakouts on key topics and a closing forum. CHWs (including promotars/es, CHRs, and MnSURE navigators), employers, educators, providers (health, social services, housing), local public health, health plans, community-based organizations, voluntary health associations, and all those committed to working for health equity are encouraged to participate.

Conference Objectives 

  1. Describe current initiatives in Minnesota that help achieve the Triple Aim of Health Equity
  2. Identify new developments, emerging lessons, best practices, and helpful resources to improve access, outcomes, and sustainability
  3. Define opportunities, research, and success factors for next-stage work to impact the health of underserved communities
  4. Strengthen cross-sector connections, celebrate progress, and recognize leadership

Be sure to register early for our one-day conference to ensure that your name is included on the participant list!

  • Register and pay online here. Online registration features immediate confirmation of your registration via email and follow-up reminders from the Minnesota Public Health Association which is serving as our conference registrar. Credit card payment is required for online registration. Please note that online payments are non-refundable and non-negotiable.
  • Register by mail and pay by check. Please complete the registration form at the link below and enclose with your check payable to Minnesota CHW Alliance. Mail check and application to:

MN CHWA Conference Rates

  • – May 26th Full Day Regular Rate: $105.00
  • – May 26th Full Day Rate for spring ’16 CHW Certificate Program Students: $45.00
  • – May 26th Afternoon-Only Rate for MPHA Members: $55.00
  • – CHW Rate for Two-Day Combined MPHA and MN CHWA Conferences: $155.00 (Note: MPHA members need to register for the MPHA conference separately at http://www.mpha.net/event-2140096)

Lodging

A block of rooms has been reserved at Country Inn & Suites at the special rate of $99.00/night plus tax (10.125%) for those who register for our conference.
